Coconino County Health and Human Services (CCHHS) launched a Community Health Needs Assessment (CHNA) survey to help identify health priorities, community needs, strengths and risk factors within Coconino County.
Community members are invited to participate and share their experiences to help shape programs and services over the upcoming years.
